This is just a guess, but I would suspect this happened the last time that
physical volume was relabeled (probably recycled).  I've seen this type of
problem a handful of times since moving to 6.1.x.  I don't know if it's
happened since we moved to 6.1.3.

If you run

        mminfo -q volid=2248550913 -r 'volume,volid,location'

and

        mminfo -q volid=2295852801 -r 'volume,volid,location'

Do you get output for both?

In your situation, I would run

        nsrim -X

and then *delete* QF2345 from your media database, and then run

        nsrim -X

again.

If any of the data on that tape is still important, you can use scanner
to repopulate the databases with the stuff you care about.  If you think
you're going to need to do that, I would generate a report about what's
on the tape before I delete it from the media database.

I wouldn't worry about fast vs slow, but I would definitely be concerned
about a query that times out.
